St. Jerome Parish Marriage Preparation
- Certificate of Baptism issued within the last six months.
- Confirmation and First Communion certificates, if available.
- Pre-Nuptial Witness forms, two each, must be notarized by priest, deacon or notary public.
- A bride or groom who has been previously married must present an official certificate of the death of the previous spouse, or a "Declaration of Nullity" from a church tribunal. Any previous marriage of either bride or groom, whether a civil or religious ceremony, must be presented to the priest and resolved by the Church.
- Certificate of Completion of a Church Marriage Preparation Program approved by the Archdiocese of Los Angeles.
- Prenuptial Inquiry Form to be completed by the bride and groom with the priest or deacon.
- Dispensation forms (e.g. interfaith marriage) to be completed with priest or deacon.
- A civil marriage license can be obtained from a local office of the registrar-County Clerk. Please note that a marriage license is good for only three months from application date. The marriage license is to be given to the priest/celebrant one week before the wedding date, at the latest
